| Canada: Hoffman-La Roche Herceptin Product Recall |

Health Canada announces that Hoffmann-La Roche Limited is recalling the following products from wholesalers and healthcare establishments because the affected lots may contain the presence of glass particulates. The affected products are:
- Perjeta-Herceptin Kit Powder for Solution (Combo pack) (Pertuzumab 420 mg/14 ml and Trastuzumab 440 mg/vial) (lot number: H0379B19)
- Herceptin 440 mg/vial Powder for Solution (Trastuzumab 440 mg/vial) (lot number: N3928B01)

In Hong Kong, Herceptin Powder & Solvent For Inj 440mg (HK-50617) and Herceptin Powder And Solvent For Injection 440mg (HK-62822) are registered pharmaceutical products containing trastuzumab. Both products are registered by Roche Hong Kong Limited (Roche), and are prescription-only medicines. As confirmed with Roche, the affected lot has not been imported into Hong Kong.
Perjeta-Herceptin Kit Powder for Solution (Combo pack) is not a registered pharmaceutical product in Hong Kong.
The Department of Health will continue to follow up with Roche on the local impact by the incident for consideration of any action deemed necessary.
