Custom Dialysis Trays/Kits contain alcohol swabs and prep pads were recalled by H & P Industries. The alcohol swabs and prep pads may not be sterile. Patients receiving hemodialysis who use the swabs and pads may be at potential risk for serious or life threatening infection. The recall includes products under the brand names of Cardinal Health, Medical Specialties, VHA, Triad, Triad Plus, North Safety, Total Resources, Atwater Carey and Lee Medical International.

In Hong Kong, the above products manufactured by H & P Industries are not registered pharmaceutical products. Similar news were released by the US FDA and posted on the website of Pharmaceutical Service on 21 March 2011, 4 May 2011, 26 August 2011, 29 August 2011 and 17 September 2011.
