Health Canada announces that Teligent Canada Inc. is recalling the following products from healthcare establishments due to visible particles observed in vials following reconstitution:
- Piperacillin and Tazobactam for injection 4.5g (lot number: 305757, 305756)
- Piperacillin and Tazobactam for injection 3.375g (lot number: 305664, 305665)
- Piperacillin and Tazobactam for injection 2.25g (lot number: 305666, 305718)

In Hong Kong, the above products are not registered pharmaceutical products.
On 17 Jan 2022, the Department of Health (DH) endorsed a licensed drug wholesaler, Luen Cheong Hong Ltd., to recall a batch of Piperacillin and Tazobactam for Injection 4.5g (batch number: 305756) from the market due to the presence of visible particles in vials following reconstitution. The product was unregistered but imported for the treatment of particular patients by the Hospital Authority. A recall statement was posted on the DH Drug Office’s website on the same date.
